When you submit an appointment request, the website may collect your name, telephone number, optional email address, requested service and location, contact preference, time preference, consent and basic technical information needed to prevent abuse.
Do not include medical history, diagnoses, medications, photographs, insurance information or other sensitive clinical information in the public website form or ordinary email. Clinical intake is handled separately.
How information is used
The practice uses submitted information to respond to appointment requests, answer questions, maintain appropriate records, protect the website and understand which outreach sources generate inquiries.
Information is not sold. It may be processed by service providers that support hosting, lead management or communications, subject to applicable agreements and safeguards.
Retention, choices and security
Information is retained only as long as reasonably needed for the stated purpose, legal obligations, dispute resolution and security. No online system can be guaranteed completely secure.
